Notice of Infringement – Claim

  1. A signature from the copyright owner or their authorized representative.
  2. Identification of the copyrighted work allegedly infringed.
  3. Identification of the material to be removed and details to help locate it.
  4. Contact information for the complaining party.
  5. A statement asserting that the use of the material is unauthorized.
  6. An affirmation of the accuracy of the notification and the authority of the complaining party.

Section 17 USC §512(f) outlines penalties for providing false information in a copyright infringement claim under 17 USC §512(c)(3).
